Enteral Formulary Management: A Cost-effective Approach

Abstract

Although enteral formularies emerged over 30 years ago, their effectiveness remains to be clearly demonstrated. Enteral formulary (EF) development has occurred over 17 years at the Toronto Rehabilitation Institute (Toronto Rehab), and a historical account is available in the literature. At Toronto Rehab, the creative process begins with a review of specific nutrition support requirements for each patient population, which is coupled with a clinical decision-making algorithm to promote standardized clinical nutrition practices and selection of products. The requirements are translated into detailed product specifications for a streamlined array of oral and tube-feeding formulae. Despite outsourced food service systems, the feasibility of simple in-house production should be considered to help curtail overall costs. The promotion of consistent practices requires nutrition support guidelines, including indications and contraindications for each EF product. Competitive bidding practices and policies that scrutinize non-formulary requests and encourage automatic substitution for non-formulary products promote formulary adherence and contain costs. Ongoing evaluation is intended to capture changes in patient populations and product availability to target opportunities for future EF revisions.

Bien que les formulaires entéraux (FE) aient vu le jour il y a plus de 30 ans, leur efficacité reste à démontrer. L’élaboration de FE date de 17 ans au Toronto Rehabilitation Institute (Toronto Rehab) et un compte rendu historique en a été publié. Au Toronto Rehab, le processus créatif commence avec un aperçu des besoins précis de soutien nutritionnel pour chaque population de patients, couplé à un algorithme de prise de décision clinique pour favoriser la normalisation des pratiques de nutrition clinique et du choix des produits. Les besoins sont traduits en spécifications de produit détaillées pour un assortiment limité de préparations buvables et de gavages. Malgré la possibilité de sous-traiter les services alimentaires, la faisabilité de la production à l'interne doit être envisagée pour réduire les coûts. La promotion de pratiques uniformes exige des directives de soutien nutritionnel, notamment les indications et contre-indications pour chaque produit de FE. Les pratiques d'appels d'offres et les politiques qui exigent l'examen des demandes de produits hors FE et encouragent la substitution automatique de ces produits favorisent l'observance des FE et diminuent les coûts. Une évaluation continue est prévue afin de déceler les changements dans les populations de patients et la disponibilité des produits pour cibler les possibilités de révision future des FE.
